Metabolic-bariatric surgery results in significant weight loss and the prevention or alleviation of obesity-related hazards and consequences. More comprehensive estimates of its influence on long-term mortality and life expectancy, particularly stratified by pre-existing diabetes status, are needed, however, to inform policy and assist patient counseling. Metabolic and bariatric surgery (MBS) has been found to improve both liver steatohepatitis and fibrosis by causing considerable and persistent weight reduction. In the management of energy balance, the hypothalamus is the primary integrator of peripheral and central inputs. Its functional significance for the efficacy of bariatric surgery is not well understood. The consequences of bariatric surgery on individuals with hypothalamic impairment might give clues.
